Abstract:

Sleep disorder is an important issue in modern society that nearly one-third of the world’s population suffers from sleep problems. However, most studies are limited to “cross-section” discussion focusing on disease screening, diagnoses or the association to other medical conditions. In the real life, for the patients with sleep disorders, it is very complicated and difficult to solve because the inconvenience and complex of diagnostic methods as well as the lack of integration and following up of therapeutic strategies. In recent years the studies show that many factors could affect sleep quality including lifestyle, dietary behavior, physical activity, even environmental pollution. Therefore, this study project is initiated by the sleep research center of Taipei Medical University (TMU) and started from the database of sleep examination from more than ten thousand patients. By the cooperation with AI and statistic researchers we will analyze the association of sleep quality and the patients’ anthropometric features, environmental factors to create the possible prediction model of sleep disorders. Second, we will conduct a prospective study to analyze the relationship of physiological parameters, environmental factors and sleep quality including the sleep test results (by polysomnography), physical activity of daily life by wearable device, voice quality and recognitive function by recording the speaking sound before sleep and just waking up, environmental factors by recording the air pollution in the bed room. Besides, the patient’s subjective feeling and sleep quality will be recorded and analyzed by chatting program and structural questionnaire of a newly developed application in the mobile phone (CBTi-Like expert chatting system). Then we can integrate these results to build up the home-based prediction model and treatment strategies for sleep disorders. Third, we will conduct a clinical trial to evaluate the treatment effects by a three month of therapeutic period. All the multidimensional and longitudinal data need to be analyzed by AI technology and knowledge domain to create a home-based, easy to use and convenient system to improve sleep quality. Finally, we will integrate the prediction model, treatment strategies and the IoT technology for controlling the environmental factors into a comprehensive system for improving the patient’s sleep and life quality.


Biography:

劉文德醫師為臺北醫學大學醫學院副教授兼睡眠研究中心研發主任,並擔任臺北醫學大學附設醫院、衛生福利部雙和醫院睡眠中心主任與胸腔內科主治醫師。同時也是該校 AIM 人工智慧醫學組織的組織發起人之一。研究專長為胸腔醫學、胸腔復健、睡眠呼吸障礙、慢性肺病監控與遠距醫療等。過去在林口長庚胸腔科系服務時,除了醫學方面的臨床研究外,於2004~2006年擔任國家實驗研究院國家高速網路與計算中心(National Center For High-Performance Computing)兼任研究員,從事跨領域的研究,結合資訊科技與醫學之創新。協助共同開發出結合網路與胸腔照護的「氣喘格網」(Asthma Grid),作為慢性氣喘病患即時監控病情與結合環境資訊(如氣象與空氣品質)的平台,爾後更進一步將此平台改進為「氣喘手機照護系統」,於2006年5月正式公開與國內電信業者(中華電信)展開合作,更於2007年International Conference of American Thoracic Society獲選為Oral Presentation,此部分研究成果於2011年刊登於國際知名期刊「歐洲呼吸雜誌(European Respiratory Journal 2011; 37(2):310-317)」。另外自2005年起同時結合手機遠距科技,開發出利用音樂節奏調整步伐的方式以應用於慢性阻塞性肺病病患居家運動訓練的系統,初步的研究成果於2006年International Conference of American Thoracic Society獲選為Oral Presentation。亦於2008年刊登於European Respiratory Journal(2008; 32(3):651-9),同時將此運動評估與訓練的方式申請多國專利,已獲得中華民國專利核准(公告日:2006/10/01);除此之外,針對慢性呼吸道病患使用之吸入性藥物裝置,亦從事改良研究,獲得國內專利「吸入劑裝置及其可紀錄用藥資料的裝置與方法」(中華民國專利, 類型:發明, 專利證書號: I310905, 公告日: 2009/06/11)。近年除了繼續推動遠距醫療,更進行穿戴式裝置應用於慢性呼吸道疾病之居家監控、睡眠呼吸模式之評估,並開始針對空氣污染與睡眠呼吸障礙之相關性進行一系列研究,獲得多項成果。目前更獲得科技部補助主持為期四年的計畫「結合人工智慧與物聯網科技發展精準睡眠醫學」(2018/1/1-2021/12/31),除此之外,這幾年來也積極地與國內外多家新創公司合作,共同開發睡眠監測的智慧醫療平台,結合穿戴式裝置、對話專家系統建立新型態睡眠醫學的整體照護模式。
